The Dangerous Goods and Hazardous Substance Form helps businesses meet their obligations under the Work Health & Safety (WHS) and health regulations in their state or territory. 

This form ensures businesses record and classify dangerous goods and substances based on their potential health risks and/or serious damage to property and the environment.  

The form includes: 

• Details of dangerous goods, including chemical details, composition/ingredients, storage location, type of goods, class number and description, CAS RN Number, UN Number and manufacturer’s MSDS 
• Storage and handling checklist, e.g. transportation, PPE, storage and disposal 
• Risk matrix
